Online Crash Reports

The Fox Valley Metro Police Department has two online options to obtain crash reports: 


  • Wisconsin Department of Transportation
    • https://crashreports.wi.gov/
    • This is the Wisconsin Department of Transportation website and only provides reportable crash reports. If your crash data is not here, you will have to use the other site. 
    • You will need one of the following to search for the report. 
      • WISDOT Document Number
      • Date and Wisconsin Driver's License number
    • Note: There is a fee to obtain crash reports through the Wisconsin DOT.

  • Wisconsin Self Reporting Crash Form
    • If law enforcement is called to a crash, then they may complete a crash report. If law enforcement does not file a crash report on your behalf, you will need to complete the Wisconsin Driver Report of a Crash DT4002.
    • Per Wisconsin Statute §346.70(1), any crash within the State of Wisconsin must be reported when it results in:
      • Injury of a person
      • $1,000.00 or more damage to any person's vehicle or property
      • Damage of $200 or more to state or other government-owned
